50/ph Job Responsibilities Follow a care plan for the patient.
Assist transfer to bed chair or toileting and daily bowel Program.
Assist patient with personal care (bathing, dressing, etc.
)Assisted manual transfer, or mechanical transfer.
Transport in lift equip van for errands or outings.
Provide light house keeping and laundry prn.
Assist with meal preparation.
Assist with liquids via feeding tube.
Assess and record all vital signs.
Document all daily activities during the shift.
Observe and report any changes with the patients condition.
